What’s the strangest rigging mishap you've seen

But used to see guys wrap slings around sharp edges — big no-no — i learned it hard when one snapped and nearly hit a coworker. Always check your gear for wear and tear; it’s not worth the risk.

Using duct tape on a sling is just asking for trouble. I saw a rigging crew try to patch a worn strap mid-job once; it cost them double in downtime when it failed. It’s crazy how some will cut corners.
